Eligibility Guidelines

  • Proof of household income (low-income identified families only)
  • Photo ID for adults
  • Proof of address (Serving zip-code 93920, 93921, 93922, 93923, 93924, 93933, 93940, 93950, 93955
  • For this program, only children who will be 12 and under during intake qualify
  • In person sign up October 14-18 (call ahead to reserve an appointment)
  • ID for children (Medi-Cal Card, School ID or Birth certificate)
