Our mobile Outreach and Engagement team, working in the rural areas of Washington County, exceeded their annual housing goal in the first seven months of the program which launched in November 2021. Their consistent “boots on the ground” direct engagement approach has thus far resulted in more than 25 individuals moving from the streets to permanent housing. This was accomplished through over 1,200 hours of case management after engaging over 200 individuals through outreach to encampments or through crisis calls received.
During these engagements the Outreach team distributed 92 hygiene kits, meals, food boxes, grocery cards and over 1,300 miscellaneous items to meet basic needs such as sleeping bags, blankets and articles of clothing. Additionally, 17 of these individuals were walked through the process of gaining identity documents and personal identification, 21 individuals were connected to healthcare and 9 households were provided with household furnishings to assist with their newly acquired living spaces. Additionally, services such as emergency hotel housing or the provision of social-emotional or spiritual support is provided as needed or requested based on the individual needs. The teams success can be directly attributed to their diligence coupled with a proven model of case management that centers a highly relational and personalized approach.
